Description

This California Department of Public Health grant provides funding to improve the quality of life and care for residents of Skilled Nursing Facilities (SNFs). Eligible applicants—including businesses, nonprofits, public agencies, and tribal governments—may propose projects such as person‑centered care initiatives, infection‑control training, arts and engagement programs, and other resident‑focused activities. Projects cannot be research‑only, capital improvements, or high‑cost technology solutions, and must not duplicate existing CMS requirements. Grants are reimbursed after receipt of invoices, with no matching‑fund requirement, and may be awarded for up to 36 months. The total pool of funding is $17.6 million, with the number and size of awards to be determined. • Eligible applicants: businesses, individuals, nonprofits, public agencies, tribal governments. • Eligible geography: applicants nationwide, but projects must involve California‑based SNFs. • Funding mechanism: reimbursement (post‑expense). • Maximum project period: 36 months. • Exclusions: pure research, capital projects, high‑cost tech, staff salary subsidies, duplicate CMS mandates.

Budget estimate

$100,000 – $5,000,000

The total grant pool is $17,602,130 and the number of awards is listed as “Dependent.” Assuming the agency distributes funds across a moderate number of projects (e.g., 5‑30 awards), individual grant amounts could range from roughly $100,000 for smaller pilot programs up to $5,000,000 for larger, multi‑year initiatives. The low end reflects modest, targeted interventions; the high end reflects comprehensive, statewide resident‑experience overhauls.
