Annual Contract for Dental PPO and Dental HMO Plan

Tarrant County is soliciting proposals for Dental PPO and Dental HMO plan administration on behalf of the Public Employee Benefits Cooperative (PEBC). The initial term of this agreement shall commence on the date of the last signatory and shall be for a period of three years. PEBC reserves the right to renew the agreement for up to two additional one-year periods. • The proposal must include all necessary documentation, including insurance certificates and a completed questionnaire response. • The bidder must identify all deficiencies and deviations from the solicitation specifications. • The bidder must agree to the terms and conditions outlined in the solicitation, including the insurance requirements and the certification of eligibility.
