The Department of Health, Division of Public Health, is seeking proposals from eligible applicants to provide services through the Tobacco Prevention and Control (Community Based Grants) Program. The program aims to prevent youth tobacco use, enhance access to tobacco cessation and substance use treatment, and promote evidence-based public health policies. There are two application groups: Community Coalition and Health Systems Change. Community Coalition grants will fund the development or expansion of community coalitions and cross-sector partnerships focused on policy, systems, and environmental change strategies to promote healthy behaviors and social norms. Health Systems Change grants will fund health systems to assess, plan, and implement system changes within healthcare settings to increase referrals and access to tobacco and substance use treatment. Key activities include conducting public education and campaigns, building community coalitions, and implementing system changes to support tobacco cessation efforts.
• The program will award up to seven Community Coalition grants and up to fourteen Health Systems Change grants.
• The total anticipated Tobacco Cessation funding for all grant awards is $3,815,000 per fiscal year.
• The program will support projects that align with program intent, demonstrate a thorough understanding of program goals and outcomes, and clearly identify the ways in which they will be achieved.
• Applicants must describe the community or health system they intend to serve, including the population targeted, the area or communities that will be served, and the community infrastructure.
• Applicants must provide a proposed budget for the first year of the project, including detail and supporting narrative.
• The program will evaluate proposals based on criteria such as the applicant's previous experience, program and administrative staffing needs, and procedures for protecting client confidentiality.
