The State of Ohio is seeking a contractor to provide hardwood flooring sanding and refinishing services at the Chillicothe District Office, specifically on the 1st floor. The contractor will be required to perform all services within the United States and provide information on the location of services and data storage. The contract will include an affirmation and disclosure form, which must be signed by the contractor. • The contractor must provide all necessary information, including the principal location of their business and the location of any subcontractors. • The contractor must agree to perform all services within the United States, unless a duly signed waiver is obtained from the State. • The contractor must disclose any changes or shifts in location of services performed by the contractor or its subcontractors before, during, and after execution of the contract.
