The Defense Health Agency (DHA) is seeking a qualified vendor to provide weekly deliveries of 400 lb of 5/8‑inch dry‑ice pellets to the 355 Medical Group Laboratory at Davis‑Monthan AFB, Tucson, AZ. Deliveries must occur every Tuesday for a full year (52 weeks), totaling 20,800 lb of dry ice. The contract is a firm‑fixed‑price, small‑business set‑aside (100% set‑aside) and will be awarded on a Lowest Price Technically Acceptable (LPTA) basis. The vendor must be an original equipment manufacturer (OEM) or an authorized dealer/distributor and must provide all required product documentation, certifications, and compliance with the Statement of Work (Attachment 1). Partial shipments are not allowed; delivery is FOB Destination to the specified loading dock. The solicitation includes option years extending the base period through 2031, but the initial award covers 1 Oct 2026 – 30 Sep 2027.
