Description
The Palm Beach County is seeking a contractor to provide hydrogen peroxide 34% for purchase and delivery. The contract will be a term contract, and the contractor must complete and correctly execute the required OSBD Schedules when responding to the solicitation and submitting payment requests after contract award. The contractor must also identify all subcontractors and subconsultants and provide detailed information about the work to be performed or materials supplied.
• The contractor must complete Schedule 1, which includes company information, contact person, and dollar amount or percentage of work for all subcontractors/subconsultants.
• The contractor must also complete Schedule 2 or 2(a) for all subcontractors/subconsultants, which includes detailed information about the work to be performed or materials supplied.
• The contractor must submit Schedule 3 with each payment request, which includes subcontracting information, approved change orders, and amounts paid to date.
• The contractor must also submit Schedule 4, which includes actual payments made to each subcontractor/subconsultant.
