National CMOP SEVELAMER CARBONATE 800MG TAB 270 DULOXETINE HCL 20MG EC CAP

The Department of Veterans Affairs is seeking a vendor to provide SEVELAMER CARBONATE 800MG TAB 270 and DULOXETINE HCL 20MG EC CAP to the Consolidated Mail Outpatient Pharmacy (CMOP). The contract will be for a period of 3 months, with multiple deliveries per month. The vendor must be registered and current in SAM.gov and provide a separate and distinct eleven-digit National Drug Code (NDC) number for each product proposed. The vendor must also comply with the provisions of the Drug Supply Chain Security Act. • The contract requires the vendor to provide bar code labeling at the unit-of-use package level, in a linear format that conforms to all GS1-128 or Health Industry Business Communication Council (HIBCC) supplier labeling standards. • The vendor must ensure that the bar code is printed clearly and consistently to meet the quality standard of Grade C or better. • The vendor must provide a firm commitment of availability and price prior to purchase order. • The vendor must comply with all applicable laws and regulations, including the Federal Food, Drug and Cosmetic Act and the Drug Supply Chain Security Act.
