Fixture Replacement YWCA Canton- Stark Co

The Ohio Department of Health has contracted Tetra Tech to test for lead in drinking water in schools and child care facilities throughout Ohio. As part of this program, initial lead in water testing was completed at five child care facilities in Stark County, and results showed that 40 faucets and fixtures had lead levels above the action level of 5 ppb. This Request for Bid is seeking a contractor to replace these fixtures with approved models that meet NSF/ANSI/CAN 61 and NSF/ANSI/CAN 372 standards. The contractor will be responsible for pre-planning and permitting, replacement of fixtures, disposal of waste, and invoice and payment. The bid package includes a list of approved fixture models and a bid form that must be completed and submitted by the deadline. • The contractor must have a valid and current Ohio Construction Industry Licensing Board (OCILB) Plumbing Contractor License and hold a Contractor’s Registration for plumbing in Stark County and/or the City of Canton. • The contractor must provide proof of liability insurance and meet the insurance requirements listed in the bid package. • The contractor must submit a bid package that includes a bid summary, bid form, and certificates of insurance. • The contractor must notify Tetra Tech of the intended replacement fixture for approval before installation and advise of any delays or changes to the schedule. • The contractor is responsible for acquiring all applicable permits prior to fixture replacement and for removal and proper disposal of any original fixtures and/or other plumbing materials.
