Employee Health Clinic Services

The City of Phoenix is seeking a contractor to provide employee health clinic services for a five-year contract period. The contractor will be responsible for establishing and operating a near-site clinic, providing same or next day medical appointments and on-site pharmacy services. The contractor must have at least five years of experience providing similar services to private and public entities comparable in size to the City of Phoenix. The contractor will be required to provide progress reports to the Human Resources, Benefits Division, and comply with the applicable Standard and Special Terms and Conditions. • The contractor must have a minimum of five consecutive years' experience providing employee health clinic services. • The contractor must have been in operation for a minimum of five consecutive years. • The contractor must provide same or next day medical appointments and on-site pharmacy services. • The contractor must establish and operate a near-site clinic. • The contractor must provide progress reports to the Human Resources, Benefits Division.
