Description
The Commonwealth of Kentucky, through its Department of Veterans Affairs, is seeking qualified physicians (or qualified entities) to provide comprehensive primary‑care services to residents of the Carl M. Brashear Radcliff Veterans Center, a 120‑bed long‑term care facility. The contract will cover scheduled and acute medical encounters, admission assessments, discharge planning, on‑call coverage, and coordination of care with Medicare, Medicaid, and VA regulations. Proposals must include a detailed technical solution, cost solution, and optional oral presentation. Vendors must submit proof of state registration, required background checks, credentialing documentation, and agree to comply with extensive federal and state healthcare regulations. The award will be a personal service contract subject to approval by the Finance and Administration Cabinet.
• Provide physician coverage (minimum one visit every 30 days for first 90 days, then every 60 days) and 24/7 on‑call support.
• Perform evaluations, orders, progress notes, discharge summaries, and death certificates.
• Ensure compliance with CMS, VA, HIPAA, and Kentucky statutes; maintain credentialing and background‑check requirements.
