Request for Application (RFA) for the 2025-2026 Diabetes Prevention Program (DPP) Subawards

The Mississippi State Department of Health (MSDH) is seeking qualified applicants for the 2025-2026 Diabetes Prevention Program (DPP) subawards. The goal is to prevent or delay the onset of type 2 diabetes among adults with prediabetes through structured, evidence-based lifestyle change interventions focused on healthy eating, physical activity, and weight loss. Awarded organizations will be expected to meet key deliverables, including enrolling eligible participants, delivering the full year-long CDC-approved curriculum, and implementing effective outreach and retention strategies. • The funding period will run from August 15, 2025, to June 29, 2027. • Up to four subgrantees will each be eligible to receive awards of up to $15,000 for two years. • Applicants must be non-federal entities, located within the state of Mississippi, and propose to establish or expand a CDC-recognized Diabetes Prevention Program (DPP). • Applicants must demonstrate the ability and commitment to serve high-risk and/or underserved populations. • Applicants must have a plan to implement the full year-long CDC-approved DPP curriculum and a commitment to pursuing or maintaining CDC recognition.
