Description
The Department of Veterans Affairs is seeking sources for the installation, setup, and commissioning of a Carl Zeiss Visulas Combi YAG/Green laser system at the Captain James A. Lovell Federal Health Care Center in North Chicago, IL. The system must meet specific requirements, including delivery, installation, testing, calibration, configuration, and verification of system performance. The vendor will also be responsible for providing staff training, maintenance overview, and laser safety overview. The system must comply with FDA guidelines, ANSI Z136.1 laser safety standards, and VA/DoD facility requirements.
• The system must include the Carl Zeiss Visulas Combi YAG/Green laser, footswitch, slit-lamp integration, eyewear, and manuals.
• The vendor must provide a minimum 12-month warranty and service response requirements.
• The vendor must be an authorized distributor for the manufacturer and provide documentation to support this.
• The vendor must indicate if they can provide domestic products equal to the Carl Zeiss Visulas Combi YAG Green Laser.
