The Government Publishing Office (acting for the Department of Energy) is seeking a vendor to produce 1,000 “Danger Locked‑Out” safety tags. The tags must be printed on white Yupo synthetic stock (≈5.1 mil) with black and Pantone 187 UV‑resistant ink, include a 3/8‑in. reinforced metal grommet, and meet stringent durability tests (marker‑resistance, tape test, 150‑line screen). Production can be performed via conventional offset or high‑resolution digital printing, but the entire order must use a single method unless prior approval is obtained. The contractor must provide a press‑quality PDF proof for approval, package and ship the finished tags using GPO Form 905, and submit invoicing with a completed W‑9 and proof of delivery.
