Description
The Indian Health Service, Inscription House Health Center (IHHC) in Tonalea, Arizona, intends to place multiple-award Blanket Purchase Agreements (BPA) with Lakota Enterprises, Inc. and First Nations Distribution, LLC for procuring various dental supplies on an as-needed basis for the Dental department. The anticipated period of performance shall be issued as a base plus two option years from the date of award with a duration of twelve (12) months. The scope of work includes providing miscellaneous disposable impression trays, temp canal enhanced, Optibond soloplus adhesive unidonse, surgifoam sponge, PPE, lollicaine, etc.
• The NAICS code assigned to this procurement is 339114 with a small business size standard of 750 Employees.
• The applicable North American Industry Classification System (NAICS) code is for Dental Laboratories.
• The prospective, multiple-award BPAs as identified in this notice are sufficient at this time.
• In accordance with Part 12.201-1(e)(3)(i)(A), establishment of multiple BPAs constitute maximum practicable competition.
