Outpatient Behavioral Healthcare Program

The County of Mohave is seeking proposals for an Outpatient Behavioral Healthcare Program to serve the needs of the community, with a priority on the justice community. The program will be located on the 2nd floor of the newly constructed Reach Out Center, which provides 3,093 square feet of space. The County expects the program to be a 24/7 operation and will not charge rent for the space. The provider will be responsible for utilities, IT requirements, and maintenance of the space. The County will provide access to the building and will be responsible for primary standard building maintenance costs. • The program must provide screening and triage to an appropriate level of care, assessment and treatment planning, and crisis intervention. • The provider must have experience and qualifications related to the scope of services, including a license from the Arizona State Department of Health to operate an outpatient behavioral healthcare program. • The provider must have a plan for staff training, professional development, and retention strategies. • The provider must have a method of approach for accomplishing the scope of services, including a narrative of their approach and details of screening and triage, assessment and treatment planning, and level of care. • The provider must have a plan for reporting and performance metrics, including monthly reports on the number of clients, justice-involved individuals, and non-justice-involved clients.
