Description
The Department of Veterans Affairs is seeking a contractor to provide brand name or equal ice machines for the St. Albans VA Medical Center. The contractor must provide seven ice and water dispensers and seven filter systems that meet the specified requirements. The ice machines must be able to dispense 25lbs of soft chewable compressed nugget ice that can produce up to 425 lbs. daily. The contractor must also provide a warranty of 3 years parts and labor with 5 years compressor parts. The contractor will be responsible for delivering the ice machines to the St. Albans VA Medical Center.
• The contractor must provide a complete manufacturer’s product specifications with all specified required elements highlighted.
• The contractor must provide a bill of materials/Cut Sheets (showing all features).
• The contractor must provide product literature.
• The contractor must provide warranty information.
• The contractor must provide a letter of authorization from the brand name or equal manufacturer of the equipment.
• The contractor must provide a completed SF1449, Block 17a, Contractor/Offeror information, including Offeror POC information (name, UEI, email, phone number; Price/Cost schedule, Blocks 30a, 30b, and 30c, signature of Offeror/Contractor, name & title of signer.
• The contractor must provide a complete FAR 52.225-4 Buy American Certificate, certifying that the equipment is Made in America.
• The contractor must provide a copy of Small Business Certification from SBA.
