Description
The Department of Veterans Affairs is seeking a qualified vendor to deliver 500 pounds of 5/8-inch dry ice pellets weekly for 52 weeks, with a total of 26,000 pounds. The dry ice must be in packaging, either in boxes or bags, and delivered to the Blood Donor Center, Processing Division, Building 133 BD-127, 3001 N Green Bay Rd., North Chicago, IL 60064. The delivery shall be every Monday prior to 12:00 CST, and if Monday is a federal holiday, the delivery will be made by 12:00 CST on Tuesday, the day after the holiday. The contract has a base year and four option years.
• The vendor must provide a quotation that includes the price, technical description of the items being quoted, and a statement specifying the extent of agreement with all terms, conditions, and provisions included in the solicitation.
• The vendor must also provide an authorization letter or other documents from the OEM verifying that the offeror is an authorized dealer, distributor, or reseller.
• The vendor must complete annual representations and certifications electronically via the System for Award Management (SAM) website.
• The award will be based on a comparative evaluation of quotes, considering factors such as price, past performance, and delivery schedule.
