Description
The Navajo Nation Department of Fire & Rescue seeks a qualified vendor to supply eight (8) LIFEPAK 35 Monitor/Defibrillator units, complete accessory kits, consumables, and a four‑year ProCare service contract, while also providing trade‑in credit for existing LIFEPAK 1000 AED devices. Bids must be submitted in sealed envelopes by the stated deadline, include required certifications, and adhere to Indian Preference requirements.
• Deliver eight LIFEPAK 35 units with batteries, chargers, electrodes, sensors, cuffs, filters, printer kits, straps, storage bags, cables, simulator, software, and cellular modem.
• Provide a four‑year ProCare service agreement and EMS Pro Tier 1 support, plus credit for trade‑in of legacy AED units.
• Submit original and two copies of the bid package, including unit pricing, Navajo Nation sales tax (6 %), shipping costs, certification of debarment/suspension status, and a completed W‑9 on company letterhead.
