Description
The Department of Veteran Affairs, Phoenix VA Health Care System is seeking a service agreement to provide preventive and repair maintenance on 187 Guldmann GH3+ ceiling mounted patient lifts. The contractor should be able to provide documentation from the OEM and have factory-trained and authorized technicians. The contractor will be responsible for preventive maintenance, repair services, labor, travel, and providing all parts. They will also be required to notify the COR, verify equipment functionality, and deliver a Field Service Report.
* The contractor must be registered in the System for Award Management (SAM) and complete Online Representations and Certifications Application (ORCA).
* The contractor must provide documentation of technical expertise and experience.
* The contractor must be able to provide preventive and repair maintenance on the Guldmann GH3+ ceiling mounted patient lifts.
* The contractor must have certified service technicians to perform services and maintenance on the equipment.
* The contractor must provide a capability statement that addresses the organization's qualifications and ability to perform as a contractor for the work described.
