Description
The Indian Health Service (IHS) is seeking a vendor to provide 5 Stryker Trurize clinical chairs with nurse cable or equal. The chairs should include various options and accessories, such as domestic packaging, no battery back-up, and optional cord reel. The vendor should be an original equipment manufacturer (OEM) authorized dealer, distributor, or reseller to ensure OEM warranty and service. The equipment should be new and not remanufactured or from the "gray market." The vendor should provide a copy of the authorized distributor letter from the manufacturer to verify their authorization. The quote should include all aspects of the RFQ, including equipment, services, and options. The government intends to award a purchase order to the responsive and responsible vendor whose conforming response provides the lowest price technically acceptable to the government.
• The equipment should be shipped FOB Destination to the Whiteriver Service Unit in Arizona.
• The vendor should be registered with the System for Award Management (SAM).
• The quote should include pricing for all line items, and the government will evaluate the price and technical acceptance of the product.
• The government may consider past performance information in the evaluation.
• The contract will include various clauses and provisions, such as those related to payment, invoicing, and subcontracting.
