Description
• Supply prosthetic and orthotic devices (e.g., limb‑sockets, braces, footwear) to MRC VR consumers statewide, billed at the EOHHS fee schedule.
• Provide professional liability insurance, CORI‑cleared staff, bilingual capabilities, and meet all federal‑fund (WIOA) and state regulatory requirements.
• Submit a comprehensive package (cover page, rate schedule, narrative proposal, resumes, business references, CORI assurance, Prompt‑Payment Discount, Certificate of Good Standing, etc.) by 11 Oct 2017 via COMMBUYS; ongoing rolling‑enrollment accepted through 31 Dec 2027.
• Contract length Jan 1 2018 – Jun 30 2028 (with one optional five‑year renewal) and annual funding up to $20 K, subject to federal appropriations.
• Evaluation based on experience, licensing, insurance, geographic coverage, language proficiency, and optional supplier‑diversity points.
