Description
The Office of State Procurement is seeking bids for the replacement and repair of two fire hydrants for the Louisiana Department of Corrections. The job is to be performed in accordance with the enclosed scope of work and specifications. A jobsite visit is required prior to bid opening, and the contractor must supply all labor, rentals, permits, and materials. The bid must include delivery of all items F.O.B. destination, and prices must be firm for the contractual period.
• The contract may be extended for 2 additional 12-month periods at the same prices, terms, and conditions.
• A bid security of 5% of the amount bid must be included with the bidder's submission.
• The contractor must comply with all applicable federal and state laws and regulations.
• The contractor must provide a completed Attachment B - Jobsite Verification Form or a signed letter from the Agency representative stating that the vendor has visited the jobsite.
• Failure to comply with the requirements may result in the bid being rejected.
