The program services will be available statewide via two options: local in-person visits or a Colorado-based telehealth model. The successful Offeror will be responsible for recruiting organizations to participate in the program, assisting in identifying program facilitators, providing support for facilitators to complete requirements for Community Health Worker certification, and tracking enrollment and participant cessation outcome data. • The program aims to promote community-based commercial tobacco/nicotine control work as identified by the Tobacco Education, Prevention and Cessation Program. • The program will provide financial incentives for program participation and will be evaluated to determine its effectiveness. • The Offeror will be required to submit a project plan, comply with STEPP onboarding procedures, and participate in STEPP meetings. • The Offeror will also be responsible for developing and completing an evaluation plan and submitting reports. • The program will be available statewide via two options: local in-person visits or a Colorado-based telehealth model.
