Family Survey Printing and Mailing Fulfillment Services

The State Council on Developmental Disabilities is seeking a contractor to provide Family Survey Printing and Mailing Fulfillment Services. The contractor will be responsible for printing, binding, inserting, and mailing surveys to eligible families. The contract is expected to last for three months, and the contractor must be able to print up to 75,000 pieces of mail. The contractor must also provide proof of insurance and meet the requirements outlined in the IFB. • The contractor must be registered with the State of California, Secretary of State, if required by law. • The contractor must submit current proof of insurance for general liability, commercial automobile insurance, and worker’s compensation. • The contractor must provide three or more references. • The contractor must meet a Disabled Veteran Business Enterprise (DVBE) participation goal of 3%. • The contractor must document the required DVBE program participation on Attachment 3, Bidder Declaration (GSPD-05-105).
