Description
The County of Los Angeles is seeking bids for full-service maintenance of a Siemens 64 CT system. The maintenance is required on an annual basis. Bidders must provide a pre-tax unit price, net of any applicable discounts, and quote F.O.B. destination, freight prepaid and allowed. The bid must include all required documentation and attachments, including a completed "Exhibit 1, Proposer’s Organization Questionnaire/Affidavit" and "Exhibit 2, Proposer’s Certification of Compliance".
• The maintenance services are for a Siemens Sensation 64 CT B157 system.
• The contract will be awarded to the most responsive and responsible bidder.
• The County reserves the right to waive any formality in the bidding or evaluation process.
• The contract will be governed by and construed in accordance with the laws of the State of California.
• The bidder must comply with all applicable federal, state, and local laws, rules, regulations, ordinances, and directives.
• The bidder must provide insurance coverage, including general liability, automobile liability, workers' compensation, and employers' liability.
• The bidder must certify that it is not debarred from federal or state health programs.
• The bidder must comply with the County's Lobbyist Ordinance and certify that it has not offered or given any gratuities to any County officer, employee, or agent.
