Description
The Department of Health & Mental Hygiene is seeking a Master Contractor to upgrade their Qualtrics software licenses to increase the allowable number of data records/responses from 20,000 to 60,000. The contractor must provide a detailed description of the required services and deliverables, including the product specifications and pricing. The agency will evaluate the proposals based on conformance to specifications, certified lowest bidder, and MSRP.

• The contractor must be an authorized reseller for the manufacturer, Qualtrics LLC/Qualtrics International.
• The contractor must provide a screenshot of the Manufacturer's Suggested Retail Price (MSRP) with the bid.
• The contractor must ensure that the proposed COTS software is virus-free.
• The contractor must furnish all agreements and terms and conditions expected to be signed or subject to in connection with the use of the products.
• The contract will be for a period of 12 months, with the option to renew.
