Description
The New Hampshire Veterans Home is seeking a qualified vendor to deliver on‑site primary‑care and medical‑director services for its resident population. The contract will cover comprehensive medical evaluation, chronic‑care management, emergency response, and coordination with pharmacy and other support staff, operating under strict VA‑mandated timelines for admissions and follow‑up. The award will be a non‑exclusive, multi‑year (up to five years with a possible two‑year extension) state contract subject to Governor/Executive Council approval.
• Provide on‑site primary‑care coverage 8 am‑1 pm, Monday‑Friday, with a minimum of two physician days per week and additional NP/PA support as needed.
• Respond to all resident calls within one hour and maintain 24/7 on‑call coverage; meet VA admission follow‑up requirements (within 72 hours of admission, then at least monthly for 90 days, thereafter every 60 days).
• Submit all required licensure, insurance, background‑check, and HIPAA compliance documentation; use the PointClick Care EHR for same‑day record entry.
• Deliver technical proposal (max 10 pages) plus financial statements, references, and certifications; proposals must be valid for 180 days.
