Rural Health Transformation Fund Grant - Mobile Units Notice of Application

The State of Vermont is seeking grant applications for Mobile Health Care Clinics to expand access to health care services in rural areas. The grant is part of the Rural Health Transformation Fund, which aims to build stronger rural health networks, improve technology and shared operations, and strengthen the rural health workforce. The purpose of the grant is to support the provision of medical, mental health, substance use disorder, and dental services to people of all ages in rural Vermont. The grant will allow for the purchase of mobile units, outfitting of mobile units, fuel and tolls, community engagement and marketing, workforce education and expansion activities, research, and analysis. Key aspects of the grant application include: • Demonstrated expertise in the provision of health, mental health, substance use disorder, and oral health services • A plan to serve Medicaid members and strengthen health outcomes and access to care for Vermont Medicaid members • A sustainability plan to maintain access to and provision of services beyond the duration of the grant • Partnerships with key stakeholders within each region the mobile clinic serves • A range of staffing models, including professional and paraprofessional staff • Provision of patient-centered care, including care coordination and referral activities to other community-based services Some key areas where funding may not be used include pre-award costs, meeting matching requirements, supplanting existing state or local funding, construction or building expansion, and clinical services that could be reimbursed by insurance.
