Description
The ALTCEW agency seeks a qualified provider to deliver mobile health services via a fully equipped “Mobile Health Assistance Van” to underserved rural communities in the Tri‑County region (Ferry, Pend Oreille, Stevens) and Whitman County, Washington. The contractor will operate the van, transport clinical equipment, staff travel, and provide health screenings without requiring participant IDs or insurance. The scope includes at least one mobile‑unit event in each county before April 30 2027, data collection limited to ZIP code, age range, and race/ethnicity, and coordination of referrals to Community Living Connections. Payments are milestone‑based (40 %‑35 %‑25 %) on a fixed‑price contract, with an administrative cost cap of 10 %.
• Provide and operate a mobile health clinic van, including equipment transport in rented cargo vehicles and staff travel in personal vehicles.
• Deliver a minimum of one mobile‑unit event in each designated county before 30 April 2027 and report limited demographic data.
• Submit separate proposals (Attachment A & B) for the Tri‑County region and Whitman County, meeting milestone documentation (vehicle specs, insurance, audit report).
