Concrete Materials and Delivery

The City of Avondale is seeking quotations for the supply and delivery of concrete materials on an as-needed basis for a period of 12 months. The contract will be awarded to the vendor with the lowest price, and the resulting contract will be effective for one year. The city reserves the right to amend the solicitation schedule as necessary. The vendor must provide pricing for every line item to be eligible for consideration. Some key points to note include: • The city requires the delivery of various types of concrete materials, including MAG A Concrete, MAG A Concrete Flowing, and 3000 PSI Fly Ash. • The vendor must state the manufacturer of each product quoted on in conformity with the specifications. • The bid price shall be made F.O.B. Destination to the designated delivery points within the city. • The city may terminate the contract as otherwise provided herein. • The contract is subject to the city's Standard Terms and Conditions, which are available on the city's website. • The vendor must provide a point of contact for follow-up questions or notifications, including name, email, and phone number. • The vendor must upload a copy of their signed W-9 using the most current W-9 form. • The vendor must download, complete, and upload the required Offer form.
