Description
The Department of Veterans Affairs is seeking a quotation for High-Low Power Exam Tables, brand name or equal. The solicitation is a 100% set-aside for small businesses. The offeror must be registered in the System for Award Management (SAM) and/or the Small Business Administration (SBA) Dynamic Small Business Search (DSBS) at the time of offer submission. The offeror must provide technical capability and pricing, as well as any additional requirements listed in the solicitation.
* The exam tables must meet the requirements specified in the solicitation.
* The offeror must provide an authorized distributor letter.
* The offeror must complete the provision 52.204-24 Representation Regarding Certain Telecommunications and Video Surveillance Services or Equipment.
* The offeror must complete the provision 52.212-3 Offeror Representations and Certification-Commercial Products and Commercial Services.
The following are some key points to consider:
• The offeror must submit a signed Standard Form (SF) 1449, Block 30a, 30b, and 30c signature page of the solicitation.
• The offeror must submit a technical volume that demonstrates the ability to provide the services.
• The offeror must submit a price quote that is specific, complete, and separate from the technical quote.
• The offeror must list the origin/country of manufacture for all products.
