UTMB Health in Galveston, Texas is seeking a qualified vendor to design, implement, and operate a comprehensive home‑based care program that includes Hospital‑at‑Home acute care, ongoing home health support, and remote patient monitoring. The contract may be for 1, 3, or 5 years and must integrate with UTMB’s Epic electronic health record, provide a full sensor suite, and support both virtual and in‑person clinical assessments. Vendors must also submit a HUB subcontracting plan if the contract value exceeds $100 k.
• Deliver acute‑care level services in patients’ residences (Hospital‑at‑Home).
• Provide nursing, therapy, medication management, and other clinical support at home.
• Supply, install, and maintain remote monitoring devices (ECG, SpO₂, BP, temperature, glucose, weight, actigraphy, spirometer) with real‑time Epic integration.
• Offer daily or as‑needed virtual/in‑person assessments, caregiver training, and 24/7 monitoring support.
• Meet TX‑RAMP certification, HIPAA/HITECH compliance, and Texas‑specific HUB subcontracting requirements.
