Description
• Estimated workload: ~900 direct therapy hours + ~100 evaluation hours (≈ 1,000 total).
• Service locations: Early Childhood Center, School 4, School 5, Middle School, High School (student loads range from 3‑10 per building).
• Mandatory qualifications: NJ‑licensed PTs, ≥5 years pediatric experience, agency approved by NJ DOE, extensive insurance (minimum $1 M professional liability, $2 M CGL, etc.).
• Proposal requirements: hourly rate schedule, full set of mandatory forms (AA questionnaire, non‑collusion affidavit, insurance certificates, background checks, etc.), submission by email and hard copy by April 29 2026 11:00 AM.
• Contract term: July 1 2026 – June 30 2027 (non‑renewable), with award expected within 60 days of receipt.
