Description

The Iowa Department of Health and Human Services is seeking qualified organizations to serve as cardiovascular health “Hub” and “Spoke” sites forming a statewide network that expands access to advanced cardiac diagnostics, interventional procedures, disease management, rehabilitation, and tele‑cardiology for rural residents. Applicants may choose among three participation models: (1) a regional Hub with at least two Spokes offering a full suite of cardiac services; (2) a single organization expanding Hub capacity in underserved rural areas; or (3) a consortium of Spokes supported by existing Hub(s) focused on preventive and chronic‑disease care. The program is funded by CMS with approximately $209 million available in the first year of a possible five‑year period, and contracts are expected to run up to five years. • Targeted services: emergency cardiac care, interventional cardiology (PCI), cardiac catheterization, advanced imaging, cardiac/vascular surgery, heart‑failure and electrophysiology, cardiac rehab, disease‑management, tele‑cardiology. • Eligible applicants: hospitals, health systems, integrated delivery networks, or consortia capable of delivering the above services and establishing formal referral agreements with Spokes by Dec 2028. • Funding: CMS‑HHS award covering up to 100 % of costs in Year 1; subsequent years subject to budget approval (anticipated award by Oct 15 2026). • Contract length: up to 5 years, with performance milestones and reporting requirements. • Geographic focus: rural Iowa (Polk County listed as example; statewide eligibility). • Submission window: 9/21/2026 – 11/17/2026.

Budget estimate

$5,000,000 – $20,000,000

The total CMS award for the first year is about $209 million, intended to be distributed among multiple Hub‑Spoke networks across the state. Assuming 10–15 major Hub contracts, an individual Hub could receive roughly $5 million to $20 million over the contract term, depending on scope, number of Spokes, and capital investment required for advanced cardiac services. Larger regional Hubs with extensive interventional capabilities would be at the higher end, while smaller, more limited Hubs would be nearer the lower end.
