The New Jersey Department of Children and Families is seeking a qualified organization to deliver the Nurse Family Partnership (NFP) evidence‑based home‑visitation model to first‑time pregnant individuals and their families in Mercer County for one fiscal year. The contractor must staff three full‑time registered‑nurse home visitors, a part‑time supervisor, and administrative support, adhere to NFP model ratios, and manage all program data through state and national systems. Funding of up to $540,838 covers operating costs and one‑time start‑up expenses, with quarterly performance reporting and a required advisory board.
• Provide weekly/bi‑weekly home visits to up to 75 families (25 per nurse) from prenatal enrollment through the child’s second birthday.
• Maintain data entry in the NFP national database and NJ Connecting NJ Link system, and submit quarterly progress and financial reports.
• Employ a supervisory structure (0.5 FTE supervisor for up to 8 nurses) and 0.25 FTE administrative support, plus convene a local advisory board and participate in state‑wide evaluation efforts.
