The Government Publishing Office (GPO) on behalf of the VA Portland Health Care System is seeking a vendor to produce 3,000 tri‑fold “Fit Test” order forms with integrated pressure‑sensitive labels. The forms are printed one‑sided in four‑color process on 70‑lb offset paper, folded, shrink‑wrapped in bundles of 50, and shipped to Vancouver, WA by May 26 2026. The contractor must provide press‑quality soft proofs, obtain approval before printing, meet strict packaging and shipping requirements, and submit invoices with proof of delivery and a completed W‑9.
• Produce 3,000 tri‑fold (8½ × 11 in.) forms with a 1½ × 1½ in. pressure‑sensitive label, one‑sided four‑color print on JCP A60 70‑lb paper.
• Provide “press quality” PDF soft proofs to VA contacts, wait for up‑to‑3‑day approval, then print, fold, shrink‑wrap (50 per bundle), and pack in ASTM‑D5118 compliant cartons.
• Ship the completed cartons to VA Portland Health Care System, Vancouver, WA by 2026‑05‑26, emailing tracking notices on the day of shipment and supplying all delivery documentation for invoicing.
