Description
The Department of Veterans Affairs is seeking a sole source contract for the provision of DiaSorin Reagents to support the Veterans Integrated Service Network 23 (VISN 23). The contract will be an Indefinite Delivery Indefinite Quantity (IDIQ) agreement for quality control kits and consumables (test kits). The contractor will be required to provide test kits, quality control kits, and other related materials. The test kits must be FDA approved and have the longest outdates possible. The contractor will also be required to provide Safety Data Sheets (SDS) and notify the laboratory regarding any recalled reagents. The contract will be for a firm fixed price, and the contractor will be responsible for shipping and replenishing the test kits as requested by the individual site laboratory.
• The contractor must provide a quarterly report to the Contracting Officer's Representative (COR) indicating the products and quantities purchased each month.
• The contractor must supply a contingency plan of services in case of a company merger and/or acquisition.
• The Government reserves the right to add additional VISN 23 Lab sites and testing kits and equipment during the life of this agreement.
• The contractor will provide direct delivery to the requesting lab at the contractor's expense.
