Description
CareOregon is seeking a risk-bearing entity to design and provide proactive population health management for members with early-stage kidney disease through End Stage Renal Disease (ESRD), provide a kidney care program, and ensure members are receiving all benefits they are entitled to. The project will begin in February 2026 and will continue for a three-year period with the option of two one-year renewals thereafter.
• The selected entity will be responsible for providing population health management services to CareOregon members.
• The entity will be required to design and implement a kidney care program.
• The entity will be responsible for ensuring that members receive all benefits they are entitled to.
• The project will involve working with CareOregon's coordinated care organizations (CCOs) and other partners to integrate physical, behavioral, and oral health care.
• The entity will be required to comply with all applicable laws and regulations, including those related to data security and confidentiality.
