AMENDMENT 1 - SCBA Physical/Fit Test and Spirometry Testing - Six River NF

The United States Department of Agriculture (USDA) is seeking a contractor to provide on-site SCBA Physicals and Spirometry Testing for employees of the Six Rivers National Forest. The testing will be conducted on 05/18/2026 and 06/15/2026 at the Six Rivers National Forest Headquarters Office. The contractor will also be required to provide local medical facility for exams for employees to travel to, on an as-needed basis during the period of performance. The contractor must meet OSHA regulations (29 CFR 1910.134(e)(1)) and provide documentation of test taken to government employees at the time of service. • The contractor will provide physicals on 05/18/2026 from 0800 to 1700 for an estimated 30 employees per day at the Six Rivers National Forest Headquarters Office. • The contractor will provide local medical facility for exams for employees to travel to, on an as-needed basis during the period of performance. • The contractor must meet OSHA regulations (29 CFR 1910.134(e)(1)). • The contractor will provide documentation of test taken to government employees at the time of service. • The contractor will provide an itemized cost and accounting summary to the Contracting Officer (CO)/Contracting Officer’s Representative (COR) for each day tests are being given.
