Description
Mohave Valley Elementary School District #16 is seeking a licensed Arizona physical therapist (or agency) to provide comprehensive therapy services for its student population during the 2025‑2026 school year. The contractor will deliver in‑person and tele‑therapy, conduct screenings and evaluations, attend IEP meetings, produce required documentation, and handle travel between the district’s three schools. All services must be covered by a single flat‑rate proposal; no hourly pricing is allowed. The district requires the contractor to hold appropriate licensure, insurance, and fingerprint clearances, and to comply with numerous state and federal regulations (HIPAA, FERPA, forced‑labor ban, etc.).
• Provide direct therapy (in‑person & tele‑therapy), screenings, evaluations, and IEP‑related meetings for approximately 10 students (≈30 minutes/week each).
• Supply all testing materials, equipment, and documentation; no billing to third‑party insurers.
• Submit a sealed bid by June 18 2025 10:30 AM, including a flat‑rate cost covering all services, required certifications, and resumes of assigned personnel.
• Contract term is 1 year, renewable up to four additional years, with potential month‑to‑month extensions.
• Must meet insurance, licensing, fingerprint, and anti‑collusion requirements; no subcontractors allowed without prior approval.
