Description
The Multnomah County Health Department, Public Health Division, Disease Prevention and Health Promotion Program, is seeking proposers to partner with to promote the health and wellbeing of the residents of Multnomah County. The program aims to reduce chronic disease disparities among communities of color, the unhoused, and other communities impacted by chronic disease. Proposers will be selected based on their experience, skills, and ability to provide services that align with the program's goals. The contract will be awarded for a period of one year, with the possibility of renewal. The proposers will be required to provide a budget plan, including personnel, hourly rates, and other pricing information. The contract will be subject to the terms and conditions outlined in the Request for Proposal.
• The program's goals include elevating shared risk and protective factors, modernizing public health systems, and maximizing collective impact for communities.
• The program will prioritize leading with race and work with communities impacted by chronic disease.
• The contract will be awarded based on a competitive process, with a focus on selecting proposers who demonstrate the desired experience, skills, and proficiency.
• The proposers will be required to provide a plan for how they will accomplish the work, including a description of their approach, personnel, and budget.
• The contract will be subject to the terms and conditions outlined in the Request for Proposal, including requirements for insurance, certification, and reporting.
