Demonstration Projects for Whole-Person Health Models for Women Living with HIV in the South

The Centers for Disease Control - NCHHSTP is seeking applications for demonstration projects to support whole-person health models for women living with HIV in the South. The goal of these projects is to integrate HIV care with broader women's health services, provide culturally and linguistically responsive services, and strengthen program coordination between clinical, community, and public health partners. Key outcomes of the project include increased initiation of rapid ART, viral suppression, screening for chronic disease and other preventive health needs, and increased coordination between clinical, community, and public health partners. • The projects will be implemented in jurisdictions with disproportionately high HIV disease burden. • The projects will provide services that are culturally and linguistically responsive. • The projects will participate in a learning collaborative to share best practices and lessons learned. • The projects will strengthen program coordination between clinical, community, and public health partners for sustainability.
