Chronic Disease Assessment, Planning and Training

The Colorado Department of Public Health and Environment (CDPHE) is seeking a qualified Offeror to assess and plan for the training of Frontline Health Professionals in chronic disease prevention and management. The project aims to improve chronic disease and cancer outcomes in rural Colorado by increasing the capacity and training of Community Health Workers (CHW), Community Health Representatives (CHR), and medical residents. The successful Offeror will provide goods and services for all elements of the project track for which they are bidding. The project has six tracks, including CHW/CHR Chronic Disease Education Assessment and Planning, CHW/CHR Workforce Development Assessment and Planning, CHW/CHR Comprehensive Communication Strategy, CHW/CHR Training Program Expansion, Colonoscopy Access Assessment and Planning, and Residency Training Program Development and Implementation for Colonoscopy/Polypectomy. • The Offeror shall complete an implementation plan and revise it annually. • The Offeror shall implement the approved plan and complete the objectives of one or more tracks. • The Offeror shall participate in the Rural Chronic Disease Consortium and complete quarterly progress reports.
