07126P0006-Homeless Outreach Services within the MTA System

The Department of Homeless Services (DHS) is seeking an appropriately qualified vendor to provide street outreach, case management, and housing placement services to individuals experiencing homelessness in public spaces, including the street and subway. The contractor will have teams go into the MTA System, engage individuals experiencing unsheltered homelessness, provide assessments, services, and placements, and case manage the clients through to permanent housing. • The target population for Homeless Services is individuals, 18 years of age or older, who are living in the MTA system. • The contractor would be responsible for ensuring that all unhoused individuals experiencing unsheltered homelessness within their catchment area have access to case management services. • The contractor would provide crisis intervention, off-site clinical back-up, and removals to hospitals by teams which would operate on a 24-hour/7-day per week basis. • The contractor would maintain a client-to-staff ratio that allows a significant and consistent amount of time to work with clients who are participating in an alternate housing search and receiving case management services.
