Description
The County of Shelby is soliciting proposals to provide a full suite of Home and Community‑Based Services (HCBS) to older adults and adults with disabilities under federal and state funding streams (OCL, OAA, FCSP). The contract is a one‑year term beginning July 1, 2026 with two optional one‑year renewals. Vendors must be licensed, insured, bonded, and capable of delivering a wide range of services—from adult day care, personal care, and homemaker assistance to caregiver training, respite (in‑home and out‑of‑home), assistive technology, pest control, and food‑box distribution. Proposals must be submitted as a hard‑copy package (original + 4 copies) plus a digital Excel scorecard on two USB drives, and must remain firm for 180 days. Evaluation will consider compliance with minimum requirements, technical capability, and price competitiveness.
• Provide in‑home and community‑based services across multiple categories (adult day care, personal care, homemaker, respite, counseling, technology, etc.).
• Must meet licensing (PSSA or home‑health), insurance, bonding, background‑check, and Title VI/E‑Verify requirements.
• Contract term July 1, 2026 – June 30, 2027 with up to two one‑year extensions; proposal deadline March 31, 2026 4 p.m. CST.
• Mandatory documentation includes extensive financial, insurance, staffing, monitoring, and customer‑satisfaction evidence (≥ 80 % satisfaction).
