The State of Vermont is seeking proposals from qualified organizations to provide e-consult technology and services to primary care and specialist healthcare providers. The purpose of this grant is to build stronger rural health networks, improve technology and shared operations, strengthen the rural health workforce, and ultimately ensure that Vermonters receive the right care at the right time for an affordable cost. The grant will support the implementation of e-consult platforms to ensure an interoperable means of access for primary care providers to consult with specialists. Some key features of the grant include:
* Eligibility is open to primary care and specialist practices physically located in Vermont, and provider consortiums that are offering shared e-consult purchasing, leasing, and/or service opportunities to primary care and specialist practices physically located in Vermont.
* Priority will be given to those serving a higher percentage of unique patients who reside in rural ZIP codes.
* The grant will support the purchase or lease of e-consult technology, as well as related hardware, system integration, and cybersecurity infrastructure.
* Technical assistance and training related to technology selection and implementation, workflow integration, cybersecurity and risk management, and operational readiness and service activation may be included in proposals.
* Proposals must demonstrate compliance with the scope and requirements of this NOFO and will be evaluated on the following criteria: qualifications/experience, comprehensiveness of proposal and implementation plan, sustainability plan, evaluation plan, cost, and rurality.
