Description
This small‑purchase RFQ requires a vendor to redesign, print, package, ship, and later collect and scan OMR evaluation forms for the IRS Nationwide Tax Forum, then produce data‑capture reports. The work includes graphic design (reducing a 5‑page Word file to a 4‑page OMR form), high‑volume printing on OCR‑compatible paper, precise packaging and timed deliveries to five conference venues, receipt and secure scanning of completed forms, and compilation of graded reports in both hard‑copy and MS Access format.
• Print 14,815 four‑page OMR forms with OCR Fiesta and black inks on specified paper, perforate, fold, and pack ≤ 32 lb cartons.
• Deliver cartons on exact dates to venues in Chicago, New Orleans, New York, Orlando, and San Diego; plus limited early‑delivery copies to Utah and California.
• Provide on‑site or remote data‑capture services: receive completed forms, scan, extract respondent data into an Access 2003 database, generate ranked‑grade reports within five work days after each forum, and a comprehensive report 30 work days after the final forum.
• Meet strict OpScan scanning specifications, privacy‑act compliance, and provide all required documentation (proofs, tracking, proof of delivery).
