Wellcheck Health Screening Services

The Montana University System (MUS) is seeking a contractor to provide wellcheck health screening services for MUS employees. The contract will be for a period of three years, with a guaranteed rate for each year. The contractor will be required to provide basic health risk assessment, blood chemistry testing, blood pressure and biometric screenings, and health promotion and education. The contractor will also be required to provide a narrative response to the attachment and upload it to the question. The evaluation criteria will be based on the scoring criteria found in Attachment 5-Evaluation Criteria. • The contractor must download and review Attachment 1-Background, Attachment 2-Scope of Services, and Attachment 3-Offeror Qualifications. • The contractor must provide a cost proposal that includes all services listed in the company's proposal. • The contractor must provide a firm rate guarantee for three consecutive years. • The contractor must comply with Executive Order No. 12-2016 promoting equal pay for Montana women. • The contractor must certify that they do not discriminate against firearm entities or firearm trade associations.
