Description
The District seeks a qualified provider to deliver comprehensive physical therapy services to its student population. The contractor will perform screenings, direct therapy (in‑person and tele‑therapy), evaluations, IEP‑related meetings, report writing, data entry, and monthly billing. All services must comply with IDEA, HIPAA, FERPA, and Arizona statutes, and the contractor must supply its own equipment, maintain required licenses and insurance, and provide fingerprint clearance for on‑site staff.
• Service scope: screenings, direct therapy, evaluations, IEP meetings, report writing, data entry, billing.
• Delivery: in‑person at three district schools, possible tele‑therapy and in‑home visits; travel time billable.
• Requirements: Arizona PT licensure, professional liability & auto insurance, fingerprint clearance, certifications (EDGAR, non‑collusion, forced‑labor ban), and monthly service logs.
• Contract: 1‑year initial term, renewable up to four additional years; net‑30 payment; no guaranteed caseload (approx. 10 students, ~30 min/week each).
• Evaluation: 30 % cost, 20 % qualifications, 15 % start‑up timing, plus several qualitative criteria.
