(RFP) Construction of EMS Station #17

The County of Henry is seeking proposals from qualified vendors to provide General Contractor services for the construction of a new 2-Bay, 5,219 sf, EMS Station located adjacent to the Heritage Senior Center. The building will include a lobby, office, day room, dining area, kitchen, locker room, bunk room, showers/restrooms, etc. The Contractor shall furnish all materials, labor, tools, equipment, and services necessary for the performance of this RFP. Some key points to consider include: * A 10% construction contingency shall be included in the base bid. * A detailed Schedule of Values for the requested services shall equal the lump sum total. * The Contractor shall submit bid prices factoring anticipation of varying economic trends and volatile costs deviations. * The Contractor shall ensure all trash generated by work performed shall be removed from the site and properly disposed as each work operation is completed. * The Contractor shall indemnify, save and hold harmless Henry County and its elected and appointed officers, employees, servants and agents from all claims, damages, lawsuits, costs and expenses. Some of the key requirements for the proposal include: • One (1) unbound clearly marked “Original,” of the proposal documents • One (1) bound complete copy (not to exceed a 1/2” capacity ring binder) identical to the original proposal documents • One (1) digital copy in PDF format on a USB flash drive identical to the original proposal documents • A completed W9 form to be submitted with the proposal • A signed and submitted form for the Supplier Inclusion Program • A signed and submitted form for the DBE Benchmark Worksheet • A signed and submitted form for the Covenant of Non-Discrimination • A signed and submitted form for the Description of Good Faith Efforts • A signed and submitted form for the Cost Proposal
